have my Permanent Account Number and have filed my returns using it. I also have the letters acknowledging my application for a PAN and a letter with the PAN number.

 What I don't have is my PAN card.

 Here is the problem. I don't remember whether I got it and lost it or if it was not delivered at all.

What should I do to get a duplicate PAN card issued?

In your case, two situations could be explored.

If you have lost your PAN card, you need to notify the police and get an FIR from your local police station. You also need to write a letter requesting the issue of a duplicate PAN card.

In case you are certain you have not got PAN card delivery, then don't do the above. In such a case, you need to fill a rectification form which is available for Rs 66 with the UTISIL branch or NSDL and attach the letter which contains your PAN number.

It could take a minimum of about 25-40 days to receive the new card. 

I lost my Form 16 of the past two years. My employer does not have a copy of it. Is that a problem?

If you have filed your income tax return for the past two years, then there is absolutely no problem if you do not have your Form 16 on record.

However, due to statutory requirements, your employer would definitely have the data of the salary of all employees for the previous financial years. So there is no reason why your payroll department should not issue a duplicate Form 16.
